Miasis refers to invasion into the living tissues of humans and other mammals by the eggs or larvae of flies from the order of Dipteria. The invasive parasitic larvae almost invariably cause massive destruction of orbital tissue especially in neglected patients accompanied by marked inflammatory reactions and secondary bacterial infections. Infestation can be caused by multiple or single larvae. We report a case of 50 yr post menopausal women who was having squamous cell carcinoma of conjunctiva and was a neglected and belonged to low socioeconomic status. The patient ultimately ended with massive orbital miasis.

Orbital miasis is very rare worldwide. Miasis is a term first introduced by F. W. Hope. Miai is a Greek word meaning ‘fly’. Miasis refers to invasion into the living tissues of humans and other mammals by the eggs or larvae of flies from the order of Dipteria [1,2]. The invasive parasitic larvae almost invariably cause massive destruction of orbital tissue especially in neglected patients accompanied by marked inflammatory reactions and secondary bacterial infections. Infestation can be caused by multiple or single larvae [3]. Nursing staff, medical and paramedic staff should be aware of this condition, particularly in high risk cases like immobile, open wound and skin malignancy patients.

Juniper Publishers - Does Intravitreal Bevacizumab Injection for Retinopathy of Prematurity Treatment Arrest Anterior Segment Development? - JOJ Ophthalmology

Purpose: To determine intravitreal bevacizumab (IVB) effect on ocular development by comparing refractive and biometric outcomes of intravitreal bevacizumab (IVB) and laser photocoagulation for treatment of retinopathy of prematurity (ROP).
Methods: A prospective nonrandomized interventional comparative study was conducted in a referral hospital for ROP management. All patients who received either single IVB or diode laser photocoagulation were enrolled. Cycloplegic refraction and biometry was performed before treatment and at the corrected age of 9 months.
Results: The IVB group included 17 patients (28 eyes; gestational age (GA): 28.54 ±2.2 weeks) and the laser group included 17 patients (34 eyes; GA: 28.53 ± 1.6 w). GA, BW and corrected age at the end of follow-up was statistically similar between the two groups. Eyes in IVB group had significantly longer axial lengths and thinner lenses at final visit (p=.037 and p=.002).
Conclusions: Following IVB treatment of ROP, eye development in general and crystalline lens in particular are less affected compared to laser treatment. This supports the idea that anterior segment arrest which was first described for laser therapy of ROP occurs minimally with IVB if at all.

Unusual Visual Presentation from Optic Neuropathy Secondary to Recurrent Oral Squamous Cell Carcinoma

A number of medical conditions may present to the emergency department involving a change or loss of vision. Many of these conditions are sudden in onset and may result in complete or partial loss of vision, such as stroke or multiple sclerosis. A 53 year old male with a past history of oral squamous cell carcinoma presented with a headache and changes in color hue in his right eye. CT imaging revealed recurrent oral squamous cell carcinoma involving the right sphenoid and ethmoid sinuses with extension into the right middle cranial fossa and orbit. It is important to note that a change in color vision may be from optic nerve dysfunction, which was found to be the case for this patient. The patient was diagnosed with optic neuropathy secondary to recurrent oral squamous cell carcinoma, which represents the first case report of such condition.

Ocular Tissue Adhesive Application in DSAEK: a Comparative Study

Idiopathic hyperammonemia is characterized by increased serum ammonia levels (>200umol/L) and is sometimes associated with normal to slightly elevated liver function tests [1]. These patients often present with encephalopathy, cerebral edema, seizures, and coma. Idiopathic hyperammonemia has been reported after high dose chemotherapy, and organ transplants including Orthotopic lung transplant [2]. Hyperammonemia is a rare, severe, and often fatal complication. There are few cases reported in the literature since 1997, including one retrospective analysis of 807 lung transplants; only 8 patients were diagnosed with hyperammonemia [3]. The exact mechanism of idiopathic hyperammonemiais not fully understood with congenital or acquired defects in ammonia metabolism, as well as bacterial infection having been described.

Ocular Health Assessment of Basic School Children in the Oforikrom Sub- Metropolis, Kumasi-Ghana

Background: Vision is critical for daily activities and sight is treasured by all. Eye diseases and disorders such as uncorrected refractive errors, cataract, glaucoma and retinopathies, tend tomar normal vision.
Purpose: To determine the prevalence of ocular conditions, their association with age and gender and the commonest associated symptoms experienced by the basic school children in the Oforikrom sub-metropolis of the Kumasi Metropolis of Ghana.
Design: A descriptive, cross sectional survey was used.
Methods: Out of the ten sub-metros in Kumasi, simple rando
m technique was used to select a sub-metropolis. The multistage sampling technique was then used to select two public primary schools in the selected sub-metropolis as well as to identify a sample of school children aged 5-16 years. The examination procedures adopted included history-taking, visual acuity testing, plus-one blur test, static retinoscopy, subjective refraction and ophthalmoscopy.
Results: A total of 500 children were examined and refractive error was found to be the most prevalent ocular condition occurring among 200 (40.0%) of the school children, followed by allergic conjunctivitis 111 (22.2%) and dry eyes 51(10.2%). The commonest symptoms recorded include itchiness (38.40%), tearing (32.4%), red eyes (30.20%) and headache (26%).
Conclusion: Uncorrected refractive error was the most prevalent ocular condition and this study indicates that the school age is a high risk group for developing refractive errors. Eye screening of school children is recommended and eye care services, especially, refractive error services should be enforced.
